Over the past few years, healthcare has been chasing efficiency and quality in services as the population continues to age and expand. The burden of chronic disease comorbidities is wide and varied at a local through to national level. Reform is happening at a national level, and we are at a pivotal moment in history for change in how we manage these comorbidities, patient-centered healthcare, the health workforce and our indigenous or CALD communities.

This year we will focus on:

• Building an integrated primary healthcare system that is patient-centric and cost/resource efficient
• Bench-mark best practice change management
• Learn how to restructure health teams (workforce efficiency and planning) to coordinate care and break down barriers between services
• Treating chronic disease to decrease clinical risk and improve patient flow
