QLD Branch LEADERSHIP FOR HEALTH PROMOTION AWARD

Call for Nominations

The QLD Branch of the Australian Health Promotion Association has developed this award
to recognise outstanding leadership in the area of health promotion.

Nominations are open to members and non – members of the Association. However, the person making the nomination must be a current member of the QLD Branch.

Selection Criteria:

The Nominator will need to complete the attached application form as well as provide
a written outline describing how the Nominee meets each of the below criteria
(max. 500 words per criteria):

• Demonstrated leadership in primary health care and health promotion that has significantly contributed to the promotion of health in Queensland
• Demonstrated commitment to the principles of primary health care and health promotion
• Demonstrated a commitment to advocating for or building capacity of the health promotion
  workforce in Queensland.

The Award recipient will receive a certificate of recognition and acknowledgement
through our QLD branch media outputs. The award recipient will be announced at the 2019 QLD Branch General Meeting.
